What You Are Authorizing
By signing SSA-827, you allow SSA and state Disability Determination Services (DDS) to obtain:
- Medical records from all providers listed
- Mental health treatment records
- School records (if applicable)
- Employment records
- Drug and alcohol treatment records
Important Things to Know
- The authorization is broad — it covers records you may not realize are relevant
